Home Health Care
Personalized health care at home.
Participants in this program receive individualized nursing, medical and rehabilitation services that allow them to maintain maximum independence and remain in the comfort of their homes.
This program is available to residents of Queens and Nassau Counties, and Brooklyn. Applications may be made after discharge from a hospital or long term care facility, or directly from home.
Home care services include:
- Comprehensive assessment by a Registered Nurse
- Individualized nursing care plans
- Physical and occupational therapy
- Speech therapy and audiology testing
- Social work counseling
- Home health aides
- Personal care workers
- Homemakers and housekeepers
- Nutritional planning
- Medical supplies and equipment
- 24-hour telephone availability
- Personal Emergency Response Systems (PERS)
Additional assistance may be available for:
- Housing modifications
- Home maintenance
- Moving services
- Social day care
- Respite care
- Congregate meals
The following services may be coordinated through this program:
- Medical transportation
- Laboratory work
- Dentistry
- Ophthalmology
- Podiatry
- Medications
- Medical day care
- Discharge planning
